For those with Custom Forms using Forms manager. What forms have your created. I’m demoing it right now and am struggling to see the use of it.

Our Facilities team purchased it to add some of the forms that they already use, but in a ticket version. It was way too clunky for us to get a good use out of it since it lacked some simple customization features. We ended up just putting in an “Info” custom field with a link to the fillable form on our website somewhere.

This is a great opportunity for our members to jump in with their use cases. I know many users who have HRSD and collect on-boarding and off-boarding information.

We have implemented a workflow using Forms Manager and Remote Support Ticketing. If a Vendor or 3rd Party needs access to one of our systems and/or VPN, they submit their request via Incident IQ and the rules attach two forms, one for VPN access, and one more general form for systems access. The ticket is then sent to the IT Director/Coordinator for Approval and processing.

I have tried several forms, each one looks good but always has something that does not work.  For instance, I duplicated a form that we have our staff sign when they “check out” a piece of equipment on loan.  I can tie it to the user but not to the device.

The other form I created was for the parents to sign permission to use the Google additional services, but it would be linked to the parent and not the student.  

Right now, I am not using forms and have recommended we not keep it next year.
